Job summary

Heart to Heart Hospice is seeking a compassionate Hospice Aide/CNA to provide direct patient care under the guidance of an RN and the care plan. You will assist patients with transfers and mobility, and document changes to ensure continuity of care.

The role requires an unencumbered CNA license and a minimum of one year in hospice or a similar setting. A valid driver’s license and reliable transportation are needed, with mileage reimbursement provided.

Qualifications

• High School Diploma or GED preferred.
• Unencumbered CNA license is required.
• At least 1 year of Hospice Aide/Nursing Assistant experience.
• Demonstrates compassionate care and integrity.
• EMR experience is a plus.

Responsibilities

• Provide direct patient care under RN supervision and Plan of Care.
• Assist with safe transfers and ambulation per policy.
• Report and document changes to ensure continuity of care.
• Practice infection control principles and maintain confidentiality.
• Communicate respectfully with patients, families, and staff.
• Submit documentation timely and accurately.
• Perform other duties as required.
• Comply with laws, Medicare regulations and policies.
