Job summary

Heart to Heart Hospice is seeking a Hospice Aide (CNA) to provide direct patient care under the direction of the RN, following the Plan of Care. You will assist with transfers, report changes, and document care to ensure continuity of care in a compassionate setting.

The role requires a current CNA license, a high school diploma or GED, and at least one year of hospice or nursing assistant experience. A driver’s license and reliable transport are also needed.

Qualifications

• Must have an unencumbered CNA license
• High School Diploma or GED preferred
• At least one year as Hospice Aide or Nursing Assistant in healthcare settings
• Demonstrates interest in welfare of ill and elderly; compassionate care
• Current driver’s license and reliable transportation
• EMR experience is a plus

Responsibilities

• Provide direct patient care under the RN according to Plan of Care
• Assist with safe transfers and ambulation per policy
• Report changes and document care to ensure continuity of care
• Follow infection control principles
• Promote respectful communication with patient/family and staff
• Timely submission of documentation
• Perform other duties as required
• Ensure compliance with laws and policies
