Job summary

Heart to Heart Hospice in Texas seeks a Certified Nurse Aide to provide direct patient care under the supervision of the RN and the Hospice Aide Plan of Care. You will assist with safe transfers, document care, and report changes to ensure continuity of care.

Requirements include a current CNA license, High School Diploma or GED, and at least one year of hospice or nursing assistant experience. A valid driver's license and EMR familiarity are a plus.

Qualifications

• High School Diploma or GED preferred.
• Unencumbered CNA license required.
• Current CNA license and certification in good standing.
• At least one year experience as Hospice Aide or Nursing Assistant.
• Demonstrates compassionate care for ill and elderly.
• High integrity and confidentiality.
• EMR experience is a plus.
• Must have a valid driver's license and reliable transportation.

Responsibilities

• Provide direct patient care under RN direction and Plan of Care.
• Assist with safe transfers and ambulation per policy.
• Report and document changes to ensure continuity of care.
• Follow infection control principles and ensure patient comfort.
• Communicate with patients, families, and staff respectfully.
• Submit documentation timely and accurately.
• Perform other duties and ensure compliance with laws and policies.
