Job summary

Moments Hospice is seeking a compassionate CNA to provide personal care and support to patients on the road in Wisconsin. You will collaborate with an RN Case Manager, perform daily living activities, and maintain patient records with a focus on comfort and dignity during challenging times.

The role includes a company car benefit, health insurance, and a supportive team environment, with training opportunities to advance clinical skills and knowledge in palliative care.

Qualifications

• CNA license is required to perform duties.
• 2Dont include extraneous text in descriptions.
• CPR certification is preferred but not mandatory.

Responsibilities

• Perform personal care activities related to daily living (bathing, dressing, grooming, toileting, skin care).
• Review patient care plans and report changes to the RN Case Manager.
• Assist with lifting and transferring patients safely.
• Assist with range of motion, positioning, exercise and ambulation.
• Assist with dining and nutrition for patients.
• Maintain electronic medical records accurately.
• Work under the direction of a Case Manager RN.
