Job summary

Transitions Hospice is seeking a compassionate CNA to provide in-home care to patients in the Springfield area. You will be part of an interdisciplinary team, delivering dignified personal care and supporting families in a home setting.

The role requires an active CNA license, CPR certification, and a willingness to travel locally. Prior hospice or home health experience is preferred, with a patient-centered approach and strong documentation habits.

Qualifications

• Active CNA license or certification in the state of employment.
• Current CPR certification.
• Preference for 1+ year hospice or home health experience.
• Valid driver’s license and willingness to travel locally.
• Ability to work with terminally ill patients and families with empathy.

Responsibilities

• Provide personal care with dignity, comfort, and respect.
• Attend to patient needs and requests promptly and compassionately.
• Monitor and record vital signs as directed.
• Observe patients and report changes to the care team.
• Communicate with patients and families in a respectful and confidential manner.
• Maintain thorough documentation in line with organizational standards.

Schedule: PRN as needed

Pay Range: $19.00 - $23.00 an hour

What You'll Do

As a Transitions Hospice CNA, you're a vital member of the interdisciplinary care team — not just a support role, but a trusted presence for patients and families. Your responsibilities include:

• Providing personal care with dignity, comfort, and respect at the center of everything
• Attending to patient needs and requests promptly and compassionately
• Monitoring and recording vital signs (temperature, pulse, respiration, blood pressure) as directed
• Observing patients and reporting changes to the care team accurately and in a timely manner
• Communicating with patients and families in a positive, ethical, and confidential manner
• Maintaining thorough documentation in line with organizational standards
• Upholding the hospice philosophy with professionalism and genuine compassion

Requirements

What You Bring

• Active CNA License or Certification in the state of employment
• Possess and maintains current CPR certification
• One (1) + year Hospice, home health or related organization experience preferred
• Comfort and sensitivity working with terminally ill patients and their families
• A calm, dependable presence under emotionally demanding circumstances
• Strong attention to detail in observation and documentation
• A valid driver's license and willingness to travel locally for patient visits
