Residential Home Health and Residential Hospice is seeking a compassionate Hospice Certified Nursing Assistant to provide in-home care in West Bloomfield, MI. This role supports a patient-centered approach with a focus on comfort, dignity, and quality of life within a home setting.

In this full-time position (40 hrs/week), you will perform ADLs, monitor changes, document care in the EMR, and support families throughout the hospice journey.

Qualifications

• Active CNA certificate in the state you work.
• High School Diploma or equivalent.
• Minimum one year healthcare experience; Hospice experience preferred.
• Current driver’s license with ability to spend ~20% driving to/from patient locations.

Responsibilities

• Deliver compassionate, patient-centered care in hospice and document care in the EMR.
• Assist patients with ADLs and monitor changes in condition.
