Del Corazon Hospice is seeking a Hospice Certified Nursing Assistant (CNA) to serve patients primarily in Santa Fe and Espanola, NM, providing care in SNFs, assisted livings, and patients' homes.

The role requires compassionate hands-on care, excellent communication, and a heart for hospice, with daily travel within the service area and a PRN schedule. Competitive pay and mileage reimbursement are offered.

Qualifications

• New Mexico CNA license required.
• Hospice or home health experience preferred.
• Valid drivers license and reliable transportation.

Responsibilities

• Provide personal care and emotional support to hospice patients in SNFs and homes.
• Assist with activities of daily living (bathing, grooming, toileting, transfers, feeding).
• Observe and report changes in patient condition to the clinical team.
• Support families with education and comfort measures.
• Maintain accurate documentation per hospice and regulatory guidelines.
• Collaborate with nurses, social workers, chaplains, and the interdisciplinary team.
• Maintain patient dignity, privacy, and comfort at all times.
• Travel throughout the service area daily.

We are seeking a Hospice Certified Nursing Assistant (CNA) to serve patients primarily in Santa Fe and Espanola, providing care in Skilled Nursing Facilities (SNFs), Assisted Livings and patients' homes. This role requires compassionate hands-on care, excellent communication, and a heart for hospice.

Key Responsibilities

• Provide personal care and emotional support to hospice patients in SNFs and private residences

• Assist with activities of daily living (bathing, grooming, toileting, transfers, feeding)

• Observe and report changes in patient condition to the clinical team

• Support families with education and comfort measures

• Maintain accurate documentation per hospice and regulatory guidelines

• Work collaboratively with nurses, social workers, chaplains, and the interdisciplinary team
