Job summary

Heritage Hospice in Albuquerque is seeking a compassionate PRN Hospice CNA to join our interdisciplinary care team. You will deliver personal care, support families, and provide comfort to patients in home or facility settings.

This role offers flexible PRN scheduling, competitive hourly pay, and a collaborative, patient‑focused environment where staff trusted to provide dignified end‑of‑life care. Travel within the coverage area is required.

Qualifications

• Current New Mexico CNA certification required.
• At least six months direct patient care experience; hospice/home health preferred.
• Reliable transportation and valid driver's license.
• Ability to work independently with professionalism and compassion.
• Strong communication and interpersonal skills; end-of-life support experience preferred.

Responsibilities

• Provide hands-on personal care to hospice patients.
• Assist with maintaining a safe, clean, and comfortable environment in home or facility.
• Observe and report changes to the RN Case Manager.
• Offer emotional support to patients and families.
• Document care accurately per standards.

We are seeking a compassionate and dedicated PRN Hospice CNA to join our interdisciplinary care team. In this role, you’ll provide personal care, comfort, and supportive services to patients and families. This position offers flexible scheduling and the opportunity to make a meaningful impact while working alongside a deeply committed hospice team.

Key Responsibilities

• Provide hands‑on personal care to hospice patients, including bathing, grooming, dressing, repositioning, and ADL support
• Assist with maintaining a safe, clean, and comfortable environment in the home or facility setting
• Observe and report changes in the patient’s condition to the RN Case Manager
• Offer emotional support to patients and families with sensitivity and respect
• Document care accurately and in accordance with regulatory and organizational standards
• Participate in team communication to ensure coordinated, high‑quality care
• Travel to patient homes, facilities, and communities within assigned coverage area

Qualifications

• Current New Mexico CNA certification
• Prefer at least six months direct patient care experience; hospice or home health experience preferred
• Reliable transportation and valid driver’s license
• Ability to work independently in home settings with professionalism and compassion
• Strong communication, time‑management, and interpersonal skills
• Comfort supporting patients and families during end‑of‑life transitions
