Job summary

Amedisys is seeking a dedicated hospice aide to support patients in their homes across Ohio. You will provide compassionate personal care, assist with daily living activities, and document changes in patient status under the plan of care.

Qualifications

• High school diploma or equivalent is required.
• Current CNA credential or registration is required.
• CPR certification is required.
• CNA experience within the last two years is preferred.
• Valid driver’s license with reliable transportation is required.

Responsibilities

• Provides personal care services to patients.
• Applies safety principles and proper body mechanics to ambulation, transfers, ROM, positioning and household chores.
• Plans and prepares nutritious meals and assists with feeding.
• Performs homemaking and other environmental tasks as assigned.
• Observes, reports, and documents changes in patient’s status and reports to the primary nurse when needed.
• Follows written care plans prepared by a registered nurse.
• Assists patients with prescribed exercises as instructed by a nurse or therapist.
• Performs and records simple urine tests for sugar, acetone, and albumin.
• Other duties as assigned.
