Job summary
Heart to Heart Hospice in Houston, TX is seeking a Hospice Aide (CNA) to provide direct patient care under the supervision of the RN and in accordance with the Plan of Care. This role supports transfers, ambulation, and helps maintain comfort for patients at life-limiting stages.
The position offers full-time hours, comprehensive benefits including Medical, Dental & Vision, PTO, and a 401k plan; a $500 sign-on bonus is available for eligible hires, and mileage reimbursement is provided for
Qualifications
- High School Diploma preferred or GED.
- Unencumbered CNA license current.
- At least one year experience as a Hospice Aide or Nursing Assistant.
- Demonstrates interest in ill and elderly care; maintains confidentiality.
- EMR experience is a plus.
- Must have valid driver’s license and reliable transportation.
Responsibilities
- Provide direct patient care under the RN and Plan of Care.
- Assist with safe transfers and ambulation per policy.
- Report and document changes and care provided for continuity of care.
- Follow infection control principles.
- Promote respectful communication with patient and family.
- Timely submission of all documentation.
- Perform other duties as required and ensure legal compliance.
