Serenity Hospice PA is seeking a compassionate Certified Nursing Assistant to join our Wyoming Valley team. This full-time role provides personalized end-of-life care in patients' homes and care facilities, with ongoing support from a dedicated interdisciplinary team.

Responsibilities include assisting with daily living activities, observing patient conditions, and reporting changes to the nurse. Current PA CNA certification and reliable transportation are required.

Qualifications

• Current Pennsylvania Certified Nursing Assistant certification.
• Hospice, home health, long-term care, or personal-care experience preferred.
• Valid driver’s license, reliable transportation, and automobile insurance.
• Ability to travel throughout the Wyoming Valley service area.
• Compassionate, dependable, and comfortable working independently.
• Strong communication and organizational skills.
• Commitment to providing respectful and exceptional end-of-life care.

Responsabilités

• Provide personal care to hospice patients in their homes and care facilities.
• Assist with bathing, grooming, dressing, toileting, and other activities of daily living.
• Provide repositioning, mobility assistance, and comfort-focused care.
• Observe and promptly report changes in a patient’s condition to the assigned nurse.
• Follow each patient’s individualized plan of care.
• Maintain accurate and timely visit documentation.
• Offer compassionate emotional support to patients and families.
• Communicate effectively with nurses and other interdisciplinary team members.
• Maintain patient dignity, privacy, safety, and comfort.
