Job summary

Paradigm Health in Indiana is seeking a Full-time Hospice CNA to deliver compassionate in-home and facility care, Monday through Friday, 8am–5pm, with territory assignments across nearby counties.

You will document patient status in EMR/EHR, support families, coordinate with the care team, and uphold HIPAA while driving to patient locations as needed. Paradigm Health emphasizes a patient-focused, team-driven culture and equal employment opportunities.

Qualifications

• 1 year CNA experience required.
• Licensed CNA in State of Indiana.
• One year Hospice experience preferred.
• Current driver’s license.
• Must have reliable transportation and insurance.

Responsibilities

• Provide compassionate care to hospice patients, ensuring comfort and dignity.
• Assist with activities of daily living (bathing, dressing, grooming).
• Monitor and document vital signs and condition changes using EMR/EHR.
• Collaborate with healthcare team to implement care plans.
• Adhere to HIPAA and maintain patient confidentiality.
• Support families by providing emotional support and education, and drive to patient locations as needed.

Job Description

• Full-time Hospice CNA
• Schedule: Monday thru Friday, 8a-5p
• Available territory openings: CNAs usually assigned to 1-3 counties near the office this position is posted to
• Provide compassionate care to hospice patients, ensuring their comfort and dignity.
