Job summary

Elara Caring is seeking a Hospice Clinical Team Manager to lead a dedicated nursing team in delivering compassionate, high-quality home-based hospice care, ensuring patients receive the right care at the right time.

You will drive clinical excellence, develop staff, and implement QAPI initiatives, reporting to the Director of Nursing or Clinical Administrator, with opportunities to shape care across our growing patient network.

Qualifications

• Bachelor’s degree preferred.
• 3 years of active RN experience, preferably in home care.
• Knowledge of Medicare/Medicaid home care benefits, policies, and procedures.
• Valid driver’s license and insurance; reliable transportation.

What is Required?

Bachelor’s degree preferred 3 years of active experience as a RN, preferably with 1 year in the home care industry Knowledge of Medicare/Medicaid home care benefits, policies, and procedures Valid driver’s license and insurance Reliable transportation to perform job tasks
