Job summary

Elara Caring is seeking a Clinical Team Manager to oversee patient care services and ensure compliance with healthcare regulations. As a part of our elite team, you'll coordinate Home Health services, review Plans of Care, and provide guidance to nursing staff.

Qualifications

As an Clinical Team Manager, you’ll contribute to our success in the following ways:

  • Oversee patient care services, staff assignments, referrals, and care coordination to ensure patient needs are consistently met.
  • Plan and coordinate Home Health services with patients, families, physicians, providers, vendors, and facilities.
  • Ensure timely start of care and ongoing compliance with applicable regulatory requirements.
  • Review and approve individualized Plans of Care, physician orders, and proposed changes for clinical appropriateness.
  • Conduct regular OASIS reviews to support accurate documentation, compliance, and quality patient care.
  • Provide guidance and coaching to direct care team members and assist with interviewing and hiring clinical staff.
  • Partner with the Branch Director or Administrator to monitor key performance metrics and support clinical operations, including field work as needed.
What is Required?
  • Associate degree in a Nursing related field required
  • 2 years home care experience as a registered nurse in a Home Health or Hospice environment
  • 1 year of supervisory experience preferred
  • Current, unrestricted RN license valid for the state of work
  • Ability to quickly become proficient in enterprise applications such as Homecare Homebase, Workday, and have knowledge in Microsoft Office
  • Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
  • Must possess a valid driver's license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations

  • Benefit eligibility can vary by position and shift status.
  • The base salary for this position is $93,000 to $100,000 annually, based on the company’s good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, and internal equity.
