Job summary

By the Bay Health is seeking a Clinical Team Manager to lead a multidisciplinary hospice/home health team in San Francisco. You will coordinate care, supervise staff, ensure patient satisfaction, and maintain regulatory standards.

The role requires a BSN or MSW, CA licensure, and several years of supervisory experience in a related setting. Hybrid work schedule with onsite and remote days is provided along with competitive pay and generous benefits.

Qualifications

• BSN in Nursing or MSW from an accredited program.
• Two years nursing or social work experience in home health, hospice or similar.
• Five years supervisory or progressive management experience.

Responsibilities

• Provide guidance to the team in regular transdisciplinary meetings and develop the patient/family plan of care.
• Assess staffing needs and redistribute resources to maximize patient care.
• Audit clinical records for regulatory compliance and document findings.
• Coordinate and supervise transdisciplinary team meetings and interventions.
• Conduct supervisory visits to assess performance per policy and standards.
