Buckeye Hospice and Palliative Care Services is seeking an experienced Hospice Clinical Supervisor (RN) to lead our Canton clinical team and ensure exceptional patient care across Ohio. The role emphasizes staff development, supervision, and compliance with hospice standards.

Qualifications

• Graduate of an accredited school of nursing.
• Current, unrestricted RN license in Ohio.
• Minimum of 2 years in home hospice or skilled home care; hospice experience preferred.
• Clinical leadership or supervisory experience preferred.
• Knowledge of hospice operations, Medicare, state regulations, ACHC standards.
• Strong clinical assessment, organizational, and decision-making skills.
• Ability to mentor and develop clinical team members.
• Excellent verbal and written communication skills.
• Professional demeanor and effective communication with patients, families, staff, and partners.
• Positive attitude, self-confidence, and commitment to organizational goals.
• Ability to work independently while collaborating with the interdisciplinary team.
• Valid driver's license, insurance, and reliable transportation.

Responsabilités

• Coordinate and oversee direct and indirect patient care services provided by clinical personnel.
• Provide supervision, guidance, and support to clinical staff to promote quality care and compliance with standards.
• Assist the Director of Clinical Services with clinical operations, staff development, and patient care oversight.
• Conduct medical record reviews and chart audits to ensure accurate documentation and quality care.
• Collect data and participate in QAPI activities.
• Review initial intake screening information to determine patient needs and service appropriateness before assigning direct care staff.
• Assist with onboarding and orientation of new clinical employees.
• Provide in-service education, coaching, and ongoing support to clinical team members.
• Conduct home visits and provide direct patient care as needed.
• Monitor medical supply needs and ensure supplies are available for patient care.
• Support compliance with Medicare requirements, Ohio regulations, and ACHC standards.
• Participate in performance evaluations and provide constructive feedback, education, and counseling to staff.
• Promote professional growth through ongoing education, training, workshops, and best practices.
• Perform additional clinical leadership responsibilities as assigned by the Director of Clinical Services.
