Job summary

Buckeye Hospice and Palliative Care Services in Medina, OH is seeking a Hospice Clinical Supervisor, RN to lead our clinical team. This leadership role coordinates patient care, supervises staff, and ensures high-quality hospice services to patients and families.

The ideal candidate is a graduate of an accredited nursing program with an active Ohio RN license, at least two years in home hospice or skilled home care, and prior supervisory experience.

Qualifications

• Graduate of an accredited school of nursing.
• Current, unrestricted Registered Nurse (RN) license in the State of Ohio.

Responsibilities

• Coordinate and oversee direct and indirect patient care services provided by clinical personnel.
• Provide supervision, guidance, and support to clinical staff to promote quality care and compliance with established standards.
• Assist the Director of Clinical Services with clinical operations, staff development, and patient care oversight.
• Conduct medical record reviews and chart audits to ensure accurate documentation and quality care.
• Collect data and participate in Quality Assurance and Performance Improvement (QAPI) activities.
• Review initial intake screening information to help determine patient needs and the appropriateness of services before assigning direct care staff.
• Assist with onboarding and orientation of new clinical employees.
• Provide in-service education, coaching, and ongoing support to clinical team members.
• Conduct home visits and provide direct patient care as needed.
• Monitor medical supply needs and help ensure appropriate supplies are available for patient care.
• Support compliance with applicable Medicare requirements, Ohio regulations, and ACHC standards.
• Participate in performance evaluations and provide constructive feedback, education, and counseling to support employee development.
• Promote professional growth through ongoing education, training, workshops, and industry best practices.
• Perform additional clinical leadership responsibilities as assigned by the Director of Clinical Services.
