Job summary

Well Care Health is seeking a Clinical Resource RN to support staffing and clinical operations across high-need home health regions. You will assess, plan, implement, and evaluate care, provide direct nursing services, case management, and coordination during vacancies and census fluctuations.

Deployment readiness and travel to assigned locations are required to uphold patient outcomes and agency standards.

Qualifications

• Graduate of an accredited nursing program (AD, Diploma, or BSN).
• Current nursing license in the state of service; multi-state licensure may be required.
• Minimum two years RN experience; Home Health experience required.
• CPR certification.
• Ability to travel across assigned locations for temporary assignments.

Responsibilities

• Provides direct nursing services and case management support per care plan.
• Performs comprehensive nursing assessments and reassessments.
• Coordinates care with physicians, families, and interdisciplinary teams.
• Adjusts schedules to support deployment needs and assignment timelines.
• Maintains continuity of care and regulatory compliance.
