Job summary

Well Care Health is seeking an experienced RN for Home Health in North Carolina. You will assess patients, develop personalized plans of care, and coordinate with physicians and families to ensure quality outcomes.

You will provide direct nursing care, manage caseloads, and travel between deployment locations while adhering to agency policies and safety standards. CPR is required and Home Health experience is preferred.

Qualifications

• Must be a licensed RN in North Carolina.
• Minimum of two years RN experience, Home Health preferred.
• CPR certification required.
• Ability to work independently and coordinate care across teams.

Responsibilities

• Performs comprehensive and ongoing nursing assessments and reassessments per program requirements.
• Develops and updates individualized care plans with physicians, patients, families, and teams.
• Provides direct nursing services and case management in accordance with orders and policies.
• Communicates changes in patient condition and treatment needs to physicians and leadership.
• Maintains scheduling and deployment readiness to meet agency targets.
• Documents all care activities accurately in medical records and complies with guidelines.
