• Graduate of an accredited nursing program (AD/ Diploma/ BSN) and active RN license.
• Minimum of two years RN experience; Home Health experience preferred.
• Current CPR certification.
• Willingness to travel across assigned Well Care Home Health locations.

Responsibilities

• Perform nursing assessments, care planning, and ongoing reassessments.
• Provide direct nursing services and case management per plan of care.
• Coordinate with physicians and interdisciplinary team regarding patient needs and changes in condition.
• Maintain documentation and communication in accordance with agency policies.
