Job description

The Clinical Resource RN is an experienced registered nurse who uses the nursing process (assesses, plans, implements, evaluates) to provide clinical staffing support across high‑need Well Care Home Health regions. This role provides direct nursing services, case management support, and care coordination during vacancies, census fluctuations, and other organizational staffing needs. The Clinical Resource RN is deployment‑ready and flexible to travel across assigned locations for assignments, while maintaining continuity of care, high‑quality patient outcomes, regulatory compliance, and adherence to agency standards.

Primary Job Duties
  1. Completes initial, comprehensive, ongoing, and reassessment activities for patients according to patient needs, program requirements, and location‑specific workflows.
  2. Develops, implements, and updates individualized care plans in collaboration with physicians, patients, families/caregivers, and interdisciplinary teams.
  3. Provides direct nursing services and case management support in accordance with the care plan, physician orders, and agency policies.
  4. Communicates with physicians, branch leadership, and interdisciplinary team members regarding patient needs, response to treatment, changes in condition, and required orders.
  5. Organizes and performs work effectively and efficiently.
  6. Maintains and adjusts schedule to support deployment needs, assignment timelines, and agency performance.
  7. Demonstrates a daily commitment to the values of the agency.
  8. Demonstrates positive interpersonal relations in dealings with all agency members.
  9. Maintains and promotes customer satisfaction.
  10. Effectively demonstrates the mission, vision, and values of the agency on a daily basis.
Quality of Work (30%)
  1. Utilizes all aspects of the Nursing Process, assessing, planning, implementing, evaluating, and documenting thorough, timely assessments, reassessments, recertification activities, and order documentation.
  2. Effectively and efficiently manages assigned patient care responsibilities, coordinates care with interdisciplinary teams, supervises aides and LPNs, maintains caseload, and communicates adverse changes to leadership and physicians.
  3. Communicates and documents information in accordance with agency policies and program standards, completing all forms accurately and documenting interdisciplinary communication.
  4. Contributes to the Clinical Resource Team and program effectiveness by incorporating interdisciplinary recommendations, demonstrating willingness to travel and accept temporary assignments, and promoting workflow improvements.
Efficiency and Effectiveness (20%)
  1. Organizes and performs work effectively and efficiently while deployed, completes required educational programs, and meets productivity expectations for patient care, documentation, and operational support.
  2. Maintains and adjusts schedule to enhance team performance and deployment readiness, ensuring timely arrival, traveling to all required counties, and maintaining continuity of care.

Job Specifications
  1. Education – Graduate of an accredited nursing program (AD, Diploma, or BSN).
  2. Licensure / Certifications – Current nursing license in the state of service; multi‑state licensure or ability to obtain licensure may be required. CPR certification required.
  3. Experience – Minimum two years of RN experience; Home Health experience required. Demonstrated ability to work independently, manage patient care responsibilities, coordinate interdisciplinary care, and support high‑need locations during temporary assignments.
  4. Technical / Motor Skills – Hand‑eye coordination for injections, use of computer/EMR, operation of patient care equipment, and ability to transfer patients.
  5. Interpersonal Skills – Ability to develop positive interactions with patients, families, physicians, hospice physicians, branch leadership, interdisciplinary team members, and staff.
  6. Physical Requirements – Ability to transfer/maneuver objects ≥50lb, push, move, lift patients, position patients, bathe, ambulate; may require reaching overhead, stair climbing, fine‑motor manipulation.
  7. Mental Abilities – Assess condition, formulate plan, select interventions, evaluate response, explain/teach, prioritize duties, learn new skills, and use supportive services.
  8. Sensory Requirements – Visual and auditory assessment for dressings, IVs, and communication.
  9. Exposure to Hazards – Noise, blood‑borne pathogens, needle puncture, infectious diseases, animal/traffic hazards, temperature changes, weather extremes.
  10. Hours of Work – Variable Monday–Friday, weekends, holidays, on‑call; flexible schedule and temporary travel assignments required.
  11. Population Served – Adults and geriatrics.
  12. Driver’s License & Vehicle – Valid license, operational vehicle, ability to travel across assigned locations, meeting agency mileage, travel, and lodging arrangements.
