Job summary

Well Care Health is seeking a Hospice Clinical Manager to lead the clinical interdisciplinary team, ensure high-quality patient care, and maintain regulatory compliance.

This role requires an NLN-accredited nursing program graduate with an active RN license and at least two years of clinical experience, including supervisory responsibilities.

The manager will mentor staff, coordinate care plans, oversee admissions, and partner with agencies to support community resources and quality standards.

Qualifications

• Graduate of NLN accredited school of nursing with current RN license.
• Minimum 2 years clinical experience; supervisory experience preferred.
• Hospice experience and knowledge of hospice regulations; strong communication skills.

Responsibilities

• Oversees daily functions of the hospice care team and serves as a resource to the interdisciplinary team.
• Directs the implementation of improved work methods to ensure admissions in line with government regulations and policy.
• Ensures agency compliance with applicable laws, regulations and accreditation standards.
• Supervises RN Case Managers, Social Workers, Chaplains, Hospice Aides, and LPNs; conducts supervisory visits in the field as needed.
• Participates in quality assessment and performance improvement initiatives.
