Job summary

The Care Team Hospice is seeking an experienced Clinical Manager (RN) to lead and support our clinical team in New Philadelphia, Ohio. This full-time role focuses on developing clinicians, ensuring quality patient care, and guiding the hospice program's success.

As Clinical Manager, you will partner with branch leadership to oversee daily operations, mentor field staff, ensure regulatory compliance, and coordinate care with physicians, patients, and families to deliver exceptional end-of-life

Qualifications

• Active RN license required.
• Minimum two years RN experience, including at least one year in hospice, home health, or related setting.
• Experience with Homecare Homebase (HCHB) preferred.
• Strong leadership, communication, and organizational skills.

Responsibilities

• Lead and support daily clinical operations while ensuring hospice regulations and quality standards.
• Mentor, educate, and develop clinical staff through coaching, orientation, and performance support.
• Oversee patient care coordination, care plan reviews, admissions, orders, and interdisciplinary collaboration to ensure outcomes.
• Monitor quality metrics, perform chart audits, and support survey readiness and performance improvement.
• Partner with leadership, physicians, referral sources, and teams to promote clinical excellence and patient/family experiences.
