Job summary

The Care Team Hospice in Akron, OH is seeking an experienced Registered Nurse to step into Clinical Team Leader leadership. You will support the day-to-day clinical operations, partner with the Branch Director, and guide nurses, aides, social workers, and chaplains to deliver compassionate hospice care.

You’ll help clinicians grow, strengthen documentation, coordinate staffing, and ensure quality care across the branch, while fostering a compassionate, innovative, and collaborative team culture.

Qualifications

• Active RN license in the state of practice required.
• Graduate of an accredited school of nursing; Associate or Bachelor's degree accepted.
• Minimum of two years RN experience within the previous 36 months.
• Hospice or Home Health experience strongly preferred.
• Previous supervisory/leadership or clinical management experience preferred.
• Current CPR certification required.

Responsibilities

• Partner with the Branch Director to oversee daily clinical operations, staffing, patient care coordination, and branch clinical performance.
• Provide leadership, coaching, education, and clinical guidance to nurses and interdisciplinary team members.
• Review patient care plans, documentation, orders, admissions, discharges, and workflows to promote quality and regulatory compliance.
• Monitor quality and performance through QAPI activities, audits, outcomes, and metrics.
• Collaborate with physicians, patients, families, referral partners, and team members to resolve concerns and enhance patient experience.
• Support clinician onboarding, competency development, performance evaluations, and survey readiness.
