Job summary

Care Initiatives Hospice - Omaha is seeking a Clinical Manager - RN to lead comprehensive hospice care, meeting patients and families wherever they feel most comfortable.

This role emphasizes compassionate, interdisciplinary collaboration across home, long-term care communities, assisted living residences, or hospital settings, delivering care with dignity and respect. Active RN license, CPR, and hospice experience are required, with CHPN preferred or obtainable within 24 months.

Qualifications

• Active RN license required in the operating state(s).
• CPR certification required.
• Hospice or palliative care experience preferred.
• At least 1 year in a leadership or charge nurse position or equivalent experience.
• 2 years of recent clinical experience in hospice, oncology, home health or public health nursing; hospice experience preferred.
• Must have home internet access.
• CHPN certification preferred or ability to obtain within 24 months.

Responsibilities

• Lead and coordinate comprehensive hospice care for patients and families.
• Collaborate with interdisciplinary teams to plan and execute patient care.
• Meet patients and families where they are most comfortable (home, LTCC, assisted living, hospital).
• Ensure care plans align with patient wishes and quality of life goals.

What it takes to be a Clinical Manager with Care Initiatives Hospice:

• Active CPR certification.
• Hospice or palliative care experience.
