Job summary

Georgia Hospice Care is seeking a devoted Hospice Clinical Liaison to coordinate referrals and build strong relationships with hospitals, physicians, and referral sources. The role involves educating healthcare professionals and community partners about hospice services, ensuring smooth transitions of care and high-quality communication.

Daily fieldwork with patients, families, and medical staff supports continuity of care and accurate documentation.

Qualifications

• Heart to serve patients and families.
• Registered Nurse, Licensed Practical Nurse, or Respiratory Therapist licensed in the state.
• 2 years of clinical care experience preferred.
• Reliable transportation and ability to lift and move as needed.

Responsibilities

• Coordinate and generate hospice referrals within existing and new accounts.
• Educate healthcare professionals, referral partners, and the community about hospice services.
• Serve as the liaison between the agency and the medical community to ensure smooth transitions of care.
• Cultivate and maintain strong relationships with hospitals, physicians, and other sources of referrals.
• Work in the field to meet with referral sources and partners across the service area.
