Job summary

AccentCare, Inc. is seeking a Hospice Clinical Liaison - RN to join our team in the Atlanta, GA area, covering Atlanta and parts of Gwinnett County.

This full-time role follows a Mon-Fri schedule with limited on-call and offers a competitive salary and benefits. As a Clinical Liaison RN, you will educate community providers, coordinate with Admissions and Team Directors, develop marketing plans, and strengthen relationships with facilities and physician groups.

Qualifications

• One year RN experience in local market with strong knowledge of home health regulations.
• Licensure as a registered nurse in the state of operation.
• Must be a licensed driver with insured vehicle.

Responsibilities

• Provide community and provider in-service education in assigned territory.
• Maintain daily communication with Admissions Department and Team Directors.
• Work with Hospice Care Consultants to develop and implement marketing plans.
• Maintain visibility with contracted inpatient facilities and physician groups.
• Act as liaison with Hospice Medical Directors and Physicians for providers and patients/families.
• Make inpatient visits at contracted facilities and communicate with Admissions Department and Team Directors.
• Provide consultation on hospice and palliative care.
