Hospice Clinical Field Staff Supervisor - RN (Healthcare)

Clinical Field Staff Supervisor / RN Case Manager
Reports to: Director of Nurses

About the Position

The RN Clinical Field Staff Supervisor / RN Case Manager is responsible for coordinating home health services for Medicare and non-Medicare patients. This role ensures the delivery of quality services in accordance with clinical and agency standards, monitors service provision, and maintains compliance with regulatory guidelines. The CFSS serves as a liaison between the community, referral sources, physicians, patients, caregivers, and agency staff.

Required Skills / Qualifications
  • Current RN license in the state of practice

Key Duties and Responsibilities
