Job summary

Hireez in Fort Lauderdale, FL seeks a Clinical Care Manager to oversee nursing teams delivering high quality pain and symptom management within a hospice setting. You will lead care planning from admission through discharge or transfer, ensure documentation accuracy, and monitor compliance with Joint Commission and internal policies.

You will supervise scheduling, 24/7 coverage, utilization reviews, and team development while collaborating with physicians to coordinate palliative care measures

Qualifications

• Requires Associate of Science in Nursing (ASN); Bachelor of Science in Nursing (BSN) preferred.
• Active RN license to practice in Florida.
• 2–3 years nursing and management experience preferred.
• Hospice nursing experience preferred.
• CHPN certification preferred; required within two years of employment.

Responsibilities

• Oversees all patient care from admission to discharge or transfer to bereavement.
• Directs care services ensuring integration within the interdisciplinary plan of care.
• Collaborates with physicians on palliative care measures for pain and symptom management.
• Ensures complete and timely data/documentation for recertification decisions.
• Manages team schedule and coordinates services 24/7.
• Performs utilization review of care levels for all patients.
