Job summary

Swan Hospice seeks a compassionate CHHA with hospice experience to join our team in New Jersey. You will provide in-home and facility-based hospice assistance, supporting residents with dignity and care alongside an experienced nursing team.

Ideal candidates hold an NJ HHA license, have at least one year of hospice experience, and possess reliable transportation to cover assigned shifts. We value empathy, punctuality, and adherence to regulatory guidelines.

Qualifications

HHA license issued in NJ.
Understanding of hospice philosophy.
Minimum 1 year experience in hospice.
Reliable transportation to attend shifts.

Responsibilities

Assist with prescribed exercises and ROM as taught by the Hospice Nurse.
Assist with toileting, positioning, transfers, and frequent turning of bed-bound residents/patients.
Provide or assist with ADLs: personal hygiene, grooming, mouth, skin care, and nails.
Assist or remind patients to take medications in compliance with regulations.
Light housekeeping of the patient’s unit/room and linens changes as needed.
Assist with meals and feeding.
Record vital signs (temperature, pulse, respiration) as supervised by nurse.
Report changes in patient condition to the Hospice Nurse and document communications.
