Job summary

Millenia Hospice is seeking a qualified Hospice Chaplain / Bereavement Coordinator to deliver spiritual care and grief support to patients, families, and caregivers. You will serve on the interdisciplinary team and oversee the bereavement program for at least 13 months post-death.

The role requires a certified Chaplain with at least one year of pastoral care experience, and a strong ability to support diverse beliefs within end-of-life care.

Qualifications

  • Must be a certified Chaplain.
  • 1 year of pastoral care or counseling experience, preferably in hospice.
  • Experience in grief counseling or bereavement support preferred.

Responsibilities

  • Conduct spiritual assessments as part of the patient assessment.
  • Develop and implement individualized spiritual care plans.
  • Provide pastoral counseling and emotional support to patients and families.
  • Facilitate prayer, sacraments, rituals, or spiritual practices as requested.
  • Coordinate with community clergy and faith leaders when appropriate.
  • Participate in interdisciplinary team meetings.
  • Provide education to hospice staff regarding spiritual care issues.
  • Document spiritual care services per hospice policies and regulatory standards.
  • Develop, implement, and maintain a bereavement program in compliance with regulations.
  • Provide grief counseling and support to families for at least 13 months following death.
  • Maintain contact with families via calls, mailings, visits, or support groups.
  • Coordinate and facilitate grief support groups and memorial services.

Position Overview

The Hospice Chaplain / Bereavement Coordinator is responsible for providing spiritual care and grief support to hospice patients, families, and caregivers in accordance with the hospice plan of care and Medicare Conditions of Participation. This role serves as a member of the interdisciplinary team (IDT), addressing spiritual needs during end-of-life care and overseeing the agency’s bereavement program, including post-death follow-up services for a minimum of 13 months.

Spiritual Care (Chaplain Role)
  • Conduct spiritual assessments as part of the comprehensive patient assessment.
  • Develop and implement individualized spiritual care plans.
  • Provide pastoral counseling and emotional support to patients and families.
  • Facilitate prayer, sacraments, rituals, or spiritual practices as requested.
  • Respect and support diverse religious, cultural, and spiritual beliefs.
  • Coordinate with community clergy and faith leaders when appropriate.
  • Participate actively in interdisciplinary team meetings.
  • Provide support and education to hospice staff regarding spiritual care issues.
  • Document all spiritual care services in accordance with hospice policies and regulatory standards.
Bereavement Services (Bereavement Coordinator Role)
  • Develop, implement, and maintain a comprehensive bereavement program in compliance with federal and state hospice regulations.
  • Conduct bereavement risk assessments and establish individualized bereavement care plans.
  • Provide grief counseling and support to families and caregivers for at least 13 months following the patient’s death.
  • Maintain regular contact with families through phone calls, mailings, visits, or support groups.
  • Coordinate and facilitate grief support groups and memorial services.
  • Provide community education regarding grief and loss when appropriate.
  • Maintain accurate documentation of bereavement services.
  • Monitor and evaluate the effectiveness of the bereavement program.
Qualifications
  • Must be a certified Chaplain
Experience
  • Minimum one (1) year of pastoral care or counseling experience, preferably in hospice or healthcare.
  • Experience in grief counseling or bereavement support strongly preferred.
Skills & Competencies
  • Strong understanding of grief processes and end-of-life dynamics.
  • Compassionate, empathetic presence.
  • Ability to work effectively within an interdisciplinary team.
  • Cultural and spiritual sensitivity.
  • Excellent communication and documentation skills.

Working Conditions
