Qualifications

• Bachelor’s degree in Divinity or related degree from an accredited seminary or institution; Master’s degree preferred.
• Ordination or commissioning/endorsement from an ordaining body recognized by the Endorser's Conference; ordination must be obtained within 12 months of hire.
• Documentation demonstrating good standing with faith group or professional affiliation with no ethical charges current or pending.
• One unit of Clinical Pastoral Education (CPE) accredited by ACPE, Inc.; or in process within six months of employment.
• Previous experience working with an EMR/EHR system preferred; appropriate computer skills required.
• Health care experience preferred.
• Mobile Driver - Valid driver's license and automobile insurance per Company policy; ability to travel.
• Must navigate stairs and spaces and endure heat; able to access homes, hospitals, nursing homes and ALFs.
• Must communicate clearly in English.

Responsibilities

• Develops a spiritual care plan in conjunction with the interdisciplinary group based upon patient/family spiritual traditions.
• Provides appropriate spiritual care and initiates interventions necessary for resolving care plan issues.
• Implements the spiritual care plan through personal contact and/or enlistment of volunteers or clergy.
• Monitors implementation of the spiritual care plan and reassesses as needed.
• Maintains documentation reflecting timeliness, completeness, and clarity of goals and plans.
• Acts as liaison between the Company and local clergy; participates in team meetings.
