Job summary

Chapters Health System in Florida seeks a Chaplain to provide spiritual care to patients and families enrolled in our hospice programs. You will collaborate with an interdisciplinary team to assess needs and develop and monitor a spiritual care plan.

The role requires ordination/endorsement, CPE, and experience with EMR/EHR systems. Willingness to travel and communicate clearly in English is essential.

Qualifications

• Bachelor’s degree in Divinity or related degree from an accredited seminary; Master’s degree preferred.
• Ordination or commissioning/endorsement from an appropriate body.
• Documentation proving good standing with faith or professional group.
• CPE credential or in process within six months of employment.

Responsibilities

• Develops a spiritual care plan in conjunction with the interdisciplinary group.
• Provides spiritual care and initiates interventions to resolve care plan issues.
• Implements the spiritual care plan with clergy or counselors as appropriate.
• Monitors and reassesses plan; maintains documentation for timeliness and accuracy.
• Acts as liaison between the Company and local clergy/congregations.
