Job summary

Heart’n Soul Hospice in Memphis, TN seeks a qualified Chaplain to provide compassionate spiritual care to hospice patients and families. The role requires advanced pastoral knowledge and experience in bereavement, crisis counseling, and collaboration with an interdisciplinary team.

Responsibilities include overseeing caseloads, conducting spiritual assessments across faith traditions, and offering counseling.

Qualifications

• Advanced pastoral knowledge via multiple pathways (theological degree, CPE, board cert, or ordination).
• Minimum one year of healthcare death and dying experience. Hospice/home health preferred.
• Valid driver’s license and reliable transportation with auto liability insurance.
• Proficiency with EMR software; FireNote preferred.
• Strong written and verbal English communication skills.

Responsibilities

• Independently manage an assigned caseload of hospice patients with scheduling and pastoral approach.
• Conduct individualized spiritual assessments for patients and families.
• Provide pastoral counseling and crisis intervention in real time.
• Participate in Interdisciplinary Group (IDG) with nursing, social work, and leadership.
• Manage bereavement support with families after patient death.
• Collaborate with clinical team to resolve spiritual and ethical issues affecting patient care.
• Maintain compliance with regulations and internal policies.
• Perform other duties as assigned.
