Job summary

Heart to Heart Hospice in Indianapolis seeks a Chaplain to provide compassionate spiritual care to patients with life-limiting illness and their families. You will manage an autonomous caseload, conduct spiritual assessments, and offer counseling across faith traditions.

You will participate in the interdisciplinary team, lead bereavement follow-up, and support ethical decision-making while adhering to regulations and agency policies. A healthcare background is preferred.

Qualifications

• Formal theological education: theology degree (Bachelor of Divinity, Master of Divinity, or equivalent).
• CPE: at least one unit (400 supervised hours) from ACPE-accredited program; four units preferred.
• Board Certification: BCC or equivalent through APC/NACC/NAJC/ACPE.
• Ordination with pastoral ministry experience: recognized religious body and minimum three years clinical pastoral ministry.

Responsibilities

• Independently manage an assigned caseload of hospice patients, determining visit frequency and pastoral approach.
• Conduct individualized spiritual assessments for patients and families across faith traditions.
• Provide professional pastoral counseling, crisis intervention, and emotional support in real time.
• Participate in Interdisciplinary Group (IDG) presenting spiritual care assessments and care plan updates.
• Manage bereavement support with families after patient death, coordinating contacts based on grief trajectory.
• Collaborate with clinical team to resolve spiritual, pastoral, and ethical issues in patient care.
• Maintain compliance with regulations and internal policies.
