Job summary

Horizon Home Care and Hospice in Milwaukee, WI, seeks a Chaplain for a full-time, first-shift role within our hospice team.

You will provide pastoral care and counseling to patients and families, perform assessments, and support spiritual well-being. The position includes an on-call rotation.

Candidates should be ordained or credentialed, with at least 3 years of spiritual care and travel readiness.

Qualifications

• Bachelor's degree from an accredited institution; Master’s degree preferred.
• Must be ordained, credentialed, or commissioned by a recognized organization.
• Minimum 3 years of spiritual/pastoral care experience, with supervised experience in medical/educational/divinity settings.
• Must obtain at least one unit of Clinical Pastoral Education (ACPE) within six months of employment or equivalency.

Responsibilities

• Provide pastoral care and counseling to hospice patients and families.
• Perform assessments, treatments and health teaching as part of spiritual care.
• Travel regularly; maintain reliable vehicle and WI driver's license.
• Participate in on-call rotation evenings, weekends, and holidays.
