Job summary

Horizon Home Care & Hospice in Wisconsin is seeking a Chaplain for a full-time role. The Chaplain will offer pastoral care and counseling to patients and families, performing assessments and health teaching as part of a multidisciplinary team.

Requirements include a bachelor’s degree, ordination or equivalent, and at least 3 years of spiritual care experience with one year in a medical or hospice setting. Willingness to travel and obtain CPCE within six months are expected.

Qualifications

• Bachelor’s degree from an accredited college or university is required; a Master’s degree is preferred.
• Ordination/credentialing from a recognized organization is required.
• Minimum 3 years of spiritual/pastoral care experience, with at least 1 year in a medical/educational/divinity setting.
• Must obtain at least one unit of Clinical Pastoral Education within six months of employment.
• Computer proficiency is essential for documentation and coordination.
• Ideally experience in hospice or grief support; willingness to travel regularly with a valid WI driver’s license.

Responsibilities

• Provide pastoral care and counseling to hospice patients and families.
• Conduct assessments, health teaching, and spiritual support as part of the care team.
• Support transitions into hospice care with dignity and self-determination.
• Participate in on-call rotations including holidays, evenings, and weekends.
