Qualifications

• Ability to speak clearly to communicate with patients and families regarding spiritual care needs.
• Literate in the English language.
• Basic counseling skills for distressed individuals.
• Ability to safely travel throughout the geographic area.
• Ability to learn computerized documentation requirements.

Responsibilities

• Provide emotional and spiritual support to patients and families in homes, facilities, and hospice settings.
• Facilitate conversations about life experiences, spiritual needs, values, and end-of-life journeys.
• Provide prayer, scripture readings, music, or spiritual practices as requested.
• Collaborate with care team, nurses, social workers, and facility staff; advocate for patients.
• Attend IDT meetings and agency meetings; support team during challenging situations.
