Job summary

Advocate Health is seeking a compassionate spiritual care provider to support Monroe-area patients and families. The role includes assessing spiritual needs, developing care plans, and providing pastoral counseling in coordination with the hospice team.

Requirements include a master's degree in theology/divinity/religious studies, 2 ACPE units, and church endorsement. Travel to homes and facilities is required; on-call weekends and potential overtime are included in a full-time schedule.

Qualifications

• Master’s degree in Theology, Divinity, or Religious Studies.
• ACPE 2 units of CPE; 5 units preferred.
• Endorsement by the appropriate church authority.

Responsibilities

• Provide initial assessment of patient’s spiritual resources and preferences; provide spiritual care and emotional support.
• Develop a spiritual plan of care with interventions and outcomes; collaborate with the hospice interdisciplinary team.
• Facilitate end-of-life conversations including goals of care and funeral arrangements.
• Collaborate with hospice team for crisis referrals.
• Participate in QA/CQI processes for Advocate Hospice.
• Travel to patient homes, assisted living, and inpatient units; 40 hours/week with on-call weekends.
