Job summary

Bristol Hospice Modesto is seeking an experienced Hospice Chaplain to assess spiritual needs, develop a patient/family care plan, and work closely with the interdisciplinary team. The role involves direct spiritual care, collaboration with community chaplains, and contributing to quality assurance efforts within a growing hospice organization.

Qualifications include seminary or theology degree (BD/M.Div. preferred), hospice experience, and proficiency with MS Word/Excel/PowerPoint and EMR

Qualifications

• Graduate of an accredited seminary or school of theology; BD, M.Div. or equivalent preferred.
• Experience working with death and dying, and with patients, families or caregivers.
• Hospice experience preferred.
• Working knowledge of Microsoft Word, Excel, PowerPoint and EMR systems.

Responsibilities

• Provide direct spiritual care to patients and families/caregivers.
• Conduct spiritual assessments and develop individualized plans of care.
• Serve as liaison with community chaplains and spiritual counselors.
• Maintain records of spiritual care services and related activities.
• Participate in interdisciplinary team meetings and care conferences.
• Recruit and support community chaplains and spiritual care volunteers.
