Job summary

Bristol Hospice in Chicago/Des Plaines, IL is seeking an experienced Hospice Chaplain to provide spiritual assessments, create individualized care plans, and deliver direct spiritual care to patients and families. You will work closely with the interdisciplinary team and community chaplains to ensure compassionate, comprehensive support.

The role requires seminary-level education (BD/M.Div. or equivalent), hospice experience preferred, and willingness to travel.

Qualifications

• Graduate of an accredited seminary or school of theology; BD, M.Div. or equivalent preferred
• Experience with death and dying in hospice care preferred
• Hospice experience preferred
• Familiarity with Microsoft Word, Excel, PowerPoint and EMR systems
• Willingness to travel to hospice locations and flexible hours

Responsibilities

• Perform spiritual assessments for patients and families in the hospice program.
• Participate in comprehensive assessments to shape individualized plans of care.
• Provide direct spiritual care to patients and families/caregivers.
• Liaise with community chaplains and spiritual counselors; build networks.
• Maintain records of spiritual care services for quality assurance and policy reviews.
• Document services and communications with chaplains and counselors.
• Attend patient care conferences as part of the interdisciplinary team.
• Offer education and support on spiritual care to the interdisciplinary group.
• Recruit community chaplains and spiritual counselors as needed and educate congregations about hospice care.
• Arrange funeral or memorial services when requested; plan memorials for staff and volunteers.
• Supervise spiritual care volunteers when assigned to patients/families.
• Participate in quality assurance and performance improvement teams.
• Perform other related duties as required.
