Job summary

Bristol Hospice is seeking an experienced Hospice Chaplain to join our Modesto team. You will perform spiritual assessments, contribute to individualized care plans, and collaborate with the Interdisciplinary Team to meet patient and family needs.

Responsibilities include direct spiritual care, liaison with community chaplains, documentation, and participation in quality improvement activities. Travel to hospice locations may be required, with flexible hours to support patient families.

Qualifications

• Graduate of an accredited seminary or theology degree.
• Experience with death and dying in patients and families.
• Hospice experience preferred.
• Willing to travel to hospice locations as assigned and be flexible with working hours

Responsibilities

• Provide spiritual care to patients and families/caregivers.
• Assist in development of the individualized plan of care through spiritual assessment.
• Document spiritual care services and maintain records for QA and policy review.
• Attend patient care conferences as part of the Interdisciplinary Team.
• Recruit community chaplains and spiritual counselors as needed.
• Coordinate funeral or memorial services for patients upon request.
