Job summary

Bristol Hospice in Cleveland, OH is seeking an experienced Hospice Chaplain to support patients and families through spiritual assessments and customized plan of care. You will work closely with the Interdisciplinary Team to tailor spiritual care and ensure timely, compassionate service.

The role requires a seminary degree (BD/M.Div.) and hospice experience; some travel to hospice locations is expected. Monday–Friday, 8 AM–5 PM with competitive benefits are offered.

Qualifications

• Graduate of an accredited seminary or school of theology, BD, M.Div. or equivalent theological degree preferred.
• Must have experience working with death and dying, individuals/family/caregiver.
• Hospice experience preferred.

Responsibilities

• Assure spiritual assessment of patients and families/caregivers in the hospice program and ensure appropriate services are rendered in a timely manner.
• Facilitate the development of the individualized plan of care by participating in the comprehensive assessment to meet identified spiritual needs.
• Provide direct spiritual care to patients and families/caregivers.
• Serve as liaison and support to community chaplains and spiritual counselors.
• Maintain records of spiritual care services utilization and related activities for quality assurance, program development, and policies and procedures review and revision.
