Job summary

Gentiva Hospice is seeking a compassionate Hospice Chaplain to provide non-denominational spiritual and emotional support to patients and families facing end-of-life care. You will work within an interdisciplinary team to assess needs and develop care plans, conducting visits, blessings, and rituals as requested.

You will support families through grief and loss, build connections with faith leaders, and document interventions and outcomes.

Qualifications

• Bachelor’s degree in theology, religion, human services, counseling, psychology, or sociology (or as required by state-specific regulations)
• Minimum of one ACPE-accredited CPE unit
• Degrees from accredited institutions preferred; AVPO approval if non-accredited may be required

Responsibilities

• Provide spiritual and emotional support to patients, families, and caregivers in alignment with beliefs and values.
• Offer non-denominational, inclusive care respecting all faiths and cultures.
• Collaborate with the interdisciplinary team in patient care conferences and with nurses, social workers, aides, and medical staff.
• Assess spiritual needs and develop individualized care plans.
• Conduct visits, prayers, blessings, and rituals as requested by patient or family.
• Assist families during grief, loss, or transition and provide bereavement support.
• Build relationships with local faith leaders and community resources for spiritual needs.
• Maintain documentation of assessments, interventions, and outcomes.

Education

Bachelor’s degree in theology, religion, human services, counseling, psychology, or sociology (or as required by state-specific regulations)

ACPE-accredited Clinical Pastoral Education (CPE)
