Life Enriching Communities is seeking a Hospice Chaplain/Bereavement Coordinator to provide spiritual guidance and support to clients, families, and staff. The role emphasizes compassionate care, bereavement services, and adherence to hospice policies and regulations.

Responsibilities include counseling, visiting homebound clients or hospitals, and coordinating bereavement plans of care for families after a death.

Qualifications

  • Master’s degree in Divinity and ordination are preferred.
  • Experience in bereavement counseling and offering pastoral care is required.
  • Strong communication and compassionate approach with families impacted by loss.

You, our Ideal Candidate

You are a Hospice Chaplain/Bereavement Coordinator that provides spiritual guidance and helps family members and significant others cope with grief and loss issues related to death. This role provides pastoral care services as well as bereavement care to clients, family members, and associates. While complying with local, state and federal laws, Medicare Hospice Conditions of Participation and Agency policies and procedures, the Chaplain/Bereavement Coordinator coordinates travel time for visits and manages a caseload to deliver care in a cost-effective manner that demonstrates an understanding of hospice reimbursement methodologies. This is a position that deals with highly sensitive issues, and requires a strong degree of compassion for others.

Responsibilities
  • Provide worship and spiritual enrichment opportunities to clients and staff. Offer Pastoral Counseling to clients and family members.
  • Attend to clients who are homebound or in local hospitals, either personally or through Friendly Visitor program.
  • For a minimum period of 13 months following the death of a hospice patient, develop, coordinate, supervise, implement, and document the provision of the Bereavement Plan of Care related to the bereavement needs of families/significant other, which include (but are not limited to) cards and letters, support groups, individual and group counseling, memorial services, and other assistance.
  • Support clients with end-of-life spiritual care. Assist with Advance Directives/Advance Care Planning.
  • Perform spiritual assessments and spiritual plans of care for clients.
  • Participate in interdisciplinary team care planning. Develop, conduct, and document bereavement training programs and continuing education for volunteers by working with Coordinator of Volunteers. Collaborate in the assignment of volunteers to bereaved family members/significant others.
  • Evaluate the effectiveness of the volunteers assigned and keep the Coordinator of Volunteers informed of their performance and any concerns or issues.
Requirements
  • Current master’s degree of Divinity, and Ministry Ordination, preferred.
  • Minimum of 2 years serving a congregation, preferably in a senior community or health care setting. Minimum of 1 years’ experience in home health or hospice preferred.
  • Minimum of 1 unit CPE, or ability to achieve within first 18 months. Experience in family therapy/counseling with issues of grief, loss, death and dying and programs development related to support and education.
  • Strong knowledge of Advanced Directives demonstrated, and well as familiarity with end-of-life processes and care. Understanding of Hospice philosophy and needs of terminally ill.
  • Board Certified Chaplaincy preferred.
  • Education in grief or loss counseling required.
  • Microsoft Office proficiency. AOD experience is a plus.
  • Valid OH driver’s license and proof of auto insurance.
