Job summary

The Care Team Home Health & Hospice is seeking a Hospice Chaplain to provide spiritual and emotional support to patients and families receiving end-of-life care in home or facility settings.

You will conduct spiritual assessments, offer counseling, and participate in interdisciplinary care planning with a compassionate, mission-driven team. A valid chaplaincy background and hospice experience are preferred; on-call coverage may be required with additional compensation.

The Hospice Chaplain provides spiritual and emotional support to hospice patients and families receiving care in home or facility settings by addressing spiritual needs, offering compassionate guidance and emotional support, and participating in interdisciplinary care planning to support quality end-of-life care.

Schedule, Hours, & Territory
Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM
Assigned coverage area determined by branch needs and patient census
Participation in an on-call rotation may be required; additional compensation is provided

Field Role Requirements
Active drivers license
Ability to pass a criminal background check
Ability to pass a Motor Vehicle Record (MVR) check
Current TB testing in accordance with agency policy
Current CPR certification as required for the role
