Job summary

The Care Team Home Health & Hospice is seeking a dedicated Hospice Chaplain to provide spiritual support to patients and families receiving end-of-life care in home or facility settings in Reading, PA. The role emphasizes compassionate presence, spiritual assessments, and collaboration with the interdisciplinary team.

You will address spiritual needs, offer counseling, and participate in care planning. On-call rotation may be required with additional compensation; valid driver's license is

Qualifications

  • Ordained, commissioned, or otherwise recognized and in good standing with a faith denomination.
  • Bachelor's degree preferred; Master of Divinity or equivalent theological education preferred.
  • Previous experience providing spiritual care in hospice, healthcare, hospital, or pastoral care.
  • Knowledge of hospice philosophy, grief support, and end-of-life spiritual care.
  • Valid driver's license, reliable transportation, and current automobile insurance required.

Job Location: Reading, PA - Reading , PA 19601Hospice Chaplain Spiritual support. Compassionate presence.

Come join a compassionate team dedicated to bringing comfort, guidance, and meaningful support to patients and families during end-of-life care. The Hospice Chaplain provides spiritual and emotional support to hospice patients and families receiving care in home or facility settings by addressing spiritual needs, offering compassionate guidance and emotional support, and participating in interdisciplinary care planning to support quality end-of-life care.

Schedule, Hours, & Territory
  • Standard operating hours are Monday through Friday, 8:0 AM to 4:30 PM
  • Reading, PA, and up to a 1-hour surrounding radius
  • Participation in an on-call rotation may be required; additional compensation is provided
