Ohioans Home Healthcare & Hospice in Toledo, OH is seeking a Part-Time Hospice Chaplain to provide spiritual care to patients and families at the end of life. The Chaplain will assess needs, coordinate with the care team, and support caregivers through difficult times.

Applicants should have a theological degree or related Master of Social Work with grief counseling qualifications and the ability to network with community clergy.

Qualifications

  • Bachelor’s degree or theological degree; experience with families and patients through death.
  • Master’s degree in Social Work with grief counseling qualifications.
  • Knowledge of hospice philosophy and service coordination.

Responsibilities

  • Assures spiritual assessment of patients and families/caregivers in the hospice program and appropriate services are rendered in a timely manner.
  • Facilitates the development of the individualized plan of care by participating in the comprehensive assessment to meet identified spiritual needs.
  • Provides direct spiritual care to patients and families/caregivers.
  • Recruits community chaplains, spiritual counselors, and volunteers adequate to meet patient and family/caregiver needs by developing community contacts and offering education through congregations about hospice care.
  • Provides for funeral or memorial services for patients as requested.
  • Plans periodic memorial services to meet the needs of personnel, volunteers, and community clergy/spiritual counselors working with the hospice team.

Ohioans Home Healthcare & Hospiceis looking for aPart TimeChaplainto help patients and family members to navigate through the end-of-life process. As aChaplain, you will be responsible for the provision of spiritual care services to patients and families/caregivers of Ohioans Home Healthcare and Hospice, either directly or through coordination of care with other spiritual counselors. We are looking for someone who can understand and empathize with the sensitive time that our patients and family members are going through while utilizing your knowledge in theology to provide them with the appropriate services.

Requirements

  • Bachelor’s degree or equivalent Theological degree and experience with families/caregivers and patients through death and/ dying issues-or- Master’s degree in Social Work with qualifications of experienced clinical grief counseling
  • Knowledge and commitment to hospice philosophy
  • Ability to network with community clergy and congregations
  • Understanding of grief response and experience with counseling individuals and/or in groups regarding loss

Join Us in Providing the Best Services to Our Patients

We will monitor medication compliance, set-up, IV therapy, injections, and provide the education necessary for administering injections.

Our team ensures compliance with special diets, weight monitoring, and the utilization of intravenous nutrition such as TPN and tube feeding.

Includes instruction on the use of inhalers, aerosol machines, oxygen, and the proper care of all equipment.

Our team provides post-operative care, offering continual monitoring of chest pain, blood pressure, edema, and education on signs and symptoms of further cardiac problems.

We conduct dressing changes, wound vac therapy, assess wound healing and identify signs of infection. We also provide dressing supplies and order wound vacs as necessary.

The team will inform patients about the disease process of chronic obstructive pulmonary disease, congestive heart failure, diabetes, renal failure, multiple sclerosis, osteoporosis, and more.

Our experienced team of pediatric clinicians have the training to care for infants and children with a variety of diagnoses and special needs. Our team performs many pediatric skills including, PICC care, tracheostomy care, chemotherapy, parenteral nutrition, nasogastric or enterostomy feedings, medication administration, providing education to parents/caregivers, and many more.
