Job summary

Ohioans Home Healthcare & Hospice is seeking a Part Time Chaplain to provide spiritual care to patients and families navigating end-of-life decisions in Sandusky and surrounding counties. You will assess spiritual needs, develop care plans, and collaborate with chaplains and volunteers to ensure timely, compassionate support.

The role emphasizes funeral/memorial services when requested, coordination with a hospice team, and ongoing staff education.

Qualifications

  • Bachelor’s degree or equivalent theological degree, with experience with families/caregivers and patients through death and dying issues.
  • Master’s degree in Social Work with qualifications of experienced clinical grief counseling preferred.
  • Knowledge and commitment to hospice philosophy.
  • Ability to network with community clergy and congregations.
  • Understanding of grief response and experience with counseling individuals and/or in groups regarding loss.

  • Facilitates the development of the individualized plan of care by participating in the comprehensive assessment to meet identified spiritual needs.
  • Provides direct spiritual care to patients and families/caregivers.
  • Recruits community chaplains, spiritual counselors, and spiritual care volunteers adequate to meet patient and family/caregiver needs by developing community contacts and offering education through congregations about hospice care.
  • Provides for funeral or memorial services for patients as requested.
  • Plans periodic memorial services to meet the needs of personnel, volunteers, and community clergy/spiritual counselors working with the hospice team.

Job description

Location: Sandusky, Seneca and Ottawa Counties

Ohioans Home Healthcare & Hospice is looking for a Part Time Chaplain to help patients and family members to navigate through the end-of-life process. As a Chaplain, you will be responsible for the provision of spiritual care services to patients and families/caregivers of Ohioans Home Healthcare & Hospice, either directly or through coordination of care with other spiritual counselors. We are looking for someone who can understand and empathize with the sensitive time that our patients and family members are going through while utilizing your knowledge in theology to provide them with the appropriate services.
