Qualifications

• Ability to work independently and travel throughout the service area.

At Ascend Home Health & Hospice, we believe every patient deserves compassionate, personalized care and support. We're looking for a caring and compassionate PRN Hospice Chaplain to support patients and families in Preston, Malad, Soda Springs, and the surrounding Southern Idaho communities.

Position Overview

The Hospice Chaplain provides spiritual and emotional support to patients and their families during end-of-life care. This role offers comfort, guidance, and a compassionate presence while respecting each patient's personal beliefs, values, and wishes.

Responsibilities

• Provide spiritual and emotional support to hospice patients and their families.
• Offer comfort and companionship during times of grief, uncertainty, and end-of-life transitions.
• Support patients and families of all faiths, beliefs, and backgrounds.
• Collaborate with the hospice care team to support each patient's individual needs.
• Document visits and patient interactions accurately.
• Provide support to families experiencing loss and grief, as appropriate.
